Metastases determine malignancy. The general attention so far was focused upon organ metastases. The molecular mechanisms thereof, while far from beeing totally elucidated, are increasingly well understood. Modern diagnostic tools now enable detection and precise localization of small lesions. In contrast, our knowledge and diagnostic capabilities regarding metastatic spread to the lymphatic system is rather limited. However, there is a number of interesting recent advances and developments. It is the aim of this conference to submit these findings to detailed analysis and discussion and to provide an outlook on future prospects.
The therapeutic relevance of an enhanced sensitivity in lymph node metastases detection has to be considered with regard to increased morbidity versus eventual prognostic improvements by adjusted therapy.An interesting concept which might improve the diagnostic accuracy while reducing operative morbidity is the �sentinel node � technique. However, surgical standards lack and many fundamental questions regarding to precise lymphatic mapping remain unanswered. Whether the concept is of general relevance or whether efficacy and applicability are limited to a very few organ sites is still undetermined. The symposium shall provide intensive discussion of these questions under methodological and tumour biological aspects. World-renowned experts with a long-time involvement in related basic and clinical research will provide us with a broad survey of actual knowledge together with a critical appraisal of actual and future developments.
